One of the most common add-on CPT codes in behavioral health is Interactive Complexity (90785). When a client encounter requires greater-than-usual effort a therapist can add this CPT code to the standard mental health billing codes (This APA guide outlines its use).

Add an interactive complexity (or any other) billing code in BreezyNotes EHR
If you do plan to add an interactive complexity code or any other billing code to an encounter, follow the steps below to add them to an encounter’s fee sheet.
- Finish writing your note and click “Save & Sign.”*
- After being redirected to the calendar, click on the notepad icon of that appointment.
- Click the “Fee Sheet” button.
- Choose CPT from the “Select Code List” box.
- Add the 90785 code.
- Click “Update” – The code should now appear in the list above.
- Justify it with the ICD10 code(s).
- Click Finish to return to the Encounter Summary.
- Click the “Submit to Clearinghouse” to queue the claim for submission to insurance.
*The Fee Sheet is finalized when you sign a note. If you modify the Fee Sheet before signing a note, it will be overwritten with the default coding.
