Forms Administration
The top section of every form shows the administrative options available based on the form type selected:
First the provider must select a CPT code – only the appropriate CPT codes for the form type may be selected (i.e. Only H2017 codes are available on the BLS, and only H0032 codes are available on the ARMHS ITP form.

- Start Time and End Time – If you select the form from the client’s chart, you will be required to choose a Start Time and End Time. BreezyNotes will automatically calculate the number of units for billing based on this.
- Date of Service – Defaults to today’s date. An MHP can change this as needed.
- Session Attendees – Can be modified as needed.
- Travel Time – Following CMS protocols, enter the travel time here. BreezyNotes will automatically add an H0046 code with the units/minutes to the Fee Sheet for you. This can only be updated if Place of Service is set to “Unassigned,” “Home” or “Other.”
- Class (if applicable) – Name of the class if applicable
- Place of Service – Options are “Office,” “Home” or “Other.” If Office is selected, no Travel Time is allowed.
- Interpreter Svcs Used – If you are billing directly for Interpreter Services, you may choose to have this turned on. When set to “Yes” an Interpreter services box opens. This requires the setup of interpreter billing codes and the addition of Interpreters in the Address Book.
Due Dates
IMPORTANT: A progress not cannot be signed if any documentation is listed as “Due Immediately.” The “Save and Sign” button will be grayed out and un-clickable:
- Next ARMHS DA Due – Shows the date the next ARMHS DA is due. This will turn red 45 days prior to the DA being due (365 days from the DA’s date of service). MHPs will be unable to Save & Sign (and therefore unable to bill) any progress notes if the ARMHS DA is “Due Immediately.”
- Next Functional Assessment Due – Shows the date the next Functional Assessment is due. This will turn red 30 days prior to the Functional Assessment’s due date (180 days from the most recent Functional Assessment’s date of service).
- Next ITP Due – Shows the date the next ARMHS Individual Treatment Plan is due OR if the ARMHS ITP does not indicate a client or guardian signature is on file. This will turn red 30 days prior to the ARMHS ITP’s due date (180 days from the most recent ARMHS ITP’s date of service).
