Rules for Signing Progress Notes

All providers must follow the rules set forth by MN DHS for completing progress notes. The client needs to:
- Have a Diagnostic Assessment that is less than one year old
- Have a WHODAS completed
- Have an MHIS form completed every 6 months. NOTE: BreezyNotes does not offer an MHIS. This must be completed on through the MN-ITS website.
- Have a Functional Assessment completed within 30 days of the Diagnostic Assessment and every 180 days thereafter
- Have an Individual Treatment Plan completed within 30 days of the Diagnostic Assessment and every 180 days thereafter
In BreezyNotes these requirements are regulated by Date of Service and signed forms. If a requirement is not met, then the Save and Sign button will be grayed out and a progress note cannot be signed.
BreezyNotes has the following forms and requirements:
ARMHS Diagnostic Assessment
- Must have a signature
- Must be less than one year old
WHODAS
- This form is available via Forms dropdown. Choose “Custom Forms.” Click Save. Then Choose WHODAS from the Select Form dropdown.
Functional Assessment
- Must have a signature
- Must be 180 or fewer days old
ARMHS Individual Treatment Plan
- Must have a signature
- Must be 180 or fewer days old
- Must have either “Client Signature on File” or “Client Guardian Signature on File” set to “Yes”
Providers have 30 days from the date of the ARMHS Diagnostic Assessment where all ARMHS Progress Notes can be signed without a valid Functional Assessment or ARMHS Individual Treatment Plan.
