The ARMHS Progress Note – CI (Community Intervention)
The ARMHS Progress Note – CI is a basic GIRP note with the same validity requirements of an ARMHS Progress Note – BLS. The CI stands for “Community Intervention” and can only bill 90882 CPT codes as such. It requires that that ARMHS Diagnostic Assessment, Functional Assessment and ARMHS Individual Treatment Plan all be valid in order to be signed.
First the provider must select a CPT code. The ARMHS Progress Note – BLS only allows you to choose H2017 codes.

- Start Time and End Time – If you select the form from the client’s chart, you will be required to choose a Start Time and End Time. BreezyNotes will automatically calculate the number of units for billing based on this.
- Date of Service – Defaults to today’s date. An MHP can change this as needed.
- Session Attendees – Can be modified as needed.
- Travel Time – Following CMS protocols, enter the travel time here. BreezyNotes will automatically add an H0046 code with the units/minutes to the Fee Sheet for you. This can only be updated if Place of Service is set to “Unassigned,” “Home” or “Other.”
- Place of Service – Options are “Office,” “Home” or “Other.” If Office is selected, no Travel Time is allowed.
- Interpreter Svcs Used – If you are billing directly for Interpreter Services, you may choose to have this turned on. When set to “Yes” an Interpreter services box opens. This requires the setup of interpreter billing codes and the addition of Interpreters in the Address Book.
No Goals or Objectives
Unlike the ARMHS Progress Note – BLS there is Goals/Objectives section. This note does not require them.
Due Dates
IMPORTANT: A progress not cannot be signed if any documentation is listed as “Due Immediately.” The “Save and Sign” button will be grayed out and un-clickable:
If you find you are unable to Save & Sign a form, look in the Due Dates section to find which form is the problem:
If you see Due Immediately next to any of the documents, use the following as a troubleshooting guide:
- Next ARMHS DA Due – The ARMHS Diagnostic Assessment is more than 365 days old, or if it’s not signed.
- Next Functional Assessment Due – The Functional Assessment is more than 180 days old or it is not signed.
- Next ITP Due – The ARMHS Individual Treatment Plan is more than 180 days old, it’s not signed OR “Client Signature on File” or “Client Guardian Signature on File” are not set to “Yes.”
