Complete a Skills Note (IEP)

To be signed a Skills Note requires a valid Evaluation and Individual Treatment Plan. Without these, the note can only be saved.
To create an Individual Skills Note:
- Click the student’s name in the list of active students to the left of the calendar OR by searching in “Clients” list (left-hand navigation)
- From the “Forms” dropdown, select Skills Note
- The skills note will include the client’s diagnosis and Goals from the treatment plan. Before you sign the note, you can update:
- Date of Service
- Start/End Times
- How many students (only applies to Group sessions)
- Goals Addressed
- Activity
- Response
- When all fields are complete, click the Save & Sign button. This form will not be editable after clicking Save & Sign.
To create a Group Skills Note:
- Drag and drop all clients to the time slot for the group session (if it’s a repeating group appointment, you can check the “Repeats” box to set up the repeating session).
- Set the length of time for the session in the appointment window
- When it’s time to write notes for the clients, click the Notepad icon () on the appointment. The DOS and Start/End Times are not editable in the skills note because they have been set in the appointment window.
- Change “How many Students” to the number of students in the session
- Complete the rest of the note.
- Click Save to save progress or Save & Sign to complete the note.
