Diagnostic Assessment

Adding a Diagnostic Assessment to a client’s record:

  • Click on your client’s name from the Active Clients list in the Calendar to go to the Client Summary.

  • In the Forms dropdown (near the top), select Diagnostic Assessment.

  • Once the form is complete click “Save & Sign”

 

There is one more step to bill for a Diagnostic Assessment

  • Drag the client’s name to his/her initial appointment time from the Active Clients area.

    • You can also click on the calendar to set up an appointment and will need to specify the client in the Client section of the appointment window

  • When the appointment window pops up, change the Category to “Intake”.

    • From here the time can be edited as well as setting a Title for the event
  • Click save.
  • On the calendar, click the Notepad icon (Notepad) on the client’s appointment and you will be taken to our diagnostic assessment form.

  • Fill out the Evaluation, including “Location and Place of Service Details” and click “Save”, “Save and Close”, “Save & Sign”, or “Save, Sign, & Submit” when done completely filling out the DA.

    • Note: Prompts with a red star are required and must be filled out.
    • Note: Once an evaluation has been signed, it cannot be changed.

 

How to Return to an Unfinished Form 

  • If the evaluation has to be completed on a different day than when it started, the you will need to update the Date of Service before Signing.
    • To return to a previous Diagnostic Assessment, go to the client whose DA needs to be finished by clicking on their name in the Active Clients list.

    • Click on “Encounter History”

    • Find the form that needs to be finished, click on it, and continue filling out
