Updating (Amending) a Form After It’s Been eSigned

After a form has been eSigned it is locked from editing. The only way to note changes is through an amendment, which appears at the end of the form as a note in the encounter summary and any reports that include the form.

  1. Find the form that you need to amend. You can do this by:
    • Finding the appointment in the calendar and clicking on the Notepad icon (Notepad).
    • Selecting the client whose form needs finalization and using the Encounter History dropdown at the top of the screen to select the office visit date
    • Clicking on “History” under Client > Encounters in the navigation to access the Past Encounter History.
  2. Above the form you need to change, click “Amend.”
  3. In the pop-up, add your amendment and click “Sign.”
